The Developmental Disabilities Administration (DDA) is requiring all DDA providers to collect and submit general ledger (GL) cost data to inform rates for traditional DDA Medicaid waiver services. The purpose of the template is to support providers in submitting data consistently across the system on an annual basis. The template will be updated to ensure consistency with DDA policies as needed.
This new portal is a web-based version of the GL Template. All data must be submitted through this site. Submissions in the form of Excel spreadsheets will not be accepted.
Once you enter your information on the next page, you will receive an email with a unique GL Template link to access the forms via the Dashboard. Be sure to save this email and link as you will need it to return to your forms to continue adding or editing data.
The FY 2026 GL data collection is required for all DDA providers who delivered and were reimbursed for traditional DDA Medicaid Waiver services to participants from July 1, 2025, to June 30, 2026. Submissions for the FY 2026 GL Template will be due on October 30, 2026.
Each organization submitting a template is required to complete and sign an attestation that the information provided is complete, accurate, and prepared in accordance with the applicable instructions.
